Prenatal care is the care you get throughout your pregnancy. Complete your first prenatal care visit on or before 14 weeks of your pregnancy or within 42 days of your enrollment date.
$30 reward for postpartum care.
Postpartum care is the care you receive after the birth of your baby to check on your physical and mental health. Complete your postpartum visit with your doctor within 7 to 84 days after having your baby.

Make sure your child is growing up healthy. Staying on top of your child’s well visits and vaccine schedule is one of the best ways to keep them healthy and safe.
$30 in rewards for childhood vaccines.
Vaccines help protect children against serious diseases. Your baby should complete both flu doses before their second birthday:
- $15 for the first flu dose; $15 for the second flu dose.
$15 reward for annual well visits for children ages 3 to 11.
Well-child visits are essential to track your child’s growth and development. Your child should have at least one well visit with their doctor every year.

Keep your teen on track with preventive care! It can have a big impact on their immediate and lifelong health.
Earn up to $50 in Foot Locker® gift cards
Sticks for Kicks (members ages 9-13). When eligible members get a qualifying HPV immunization “stick” (shot), they’ll earn a $25 gift card to buy “kicks” (shoes) and/or apparel at Foot Locker. If they get another qualifying stick, they can earn a second $25 Foot Locker gift card (up to $50 total).
$25 reward for well visits for ages 12 to 17.

Up to $65 in rewards for adult preventive care.
By preventing problems, or identifying them early, you are more likely to live a longer, healthier life. Talk to your health care provider to find out which screenings are right for you – based on your age, gender, health status and family history.
$25 reward for adult preventive visit.**
Health care visits are an opportunity for individuals to receive preventive services and counseling on topics such as diet and exercise. These visits also can help them to address acute issues or manage chronic conditions. Adults ages 20 years and older should have an annual in-person or telehealth preventive visit with their provider.
$25 reward for breast cancer screening.
A mammogram is a safe, low-dose X-ray and the best way to detect and diagnose breast cancer. Women ages 50 to 74 should have an annual mammogram.
$15 reward for diabetes HbA1c testing.
The A1C test a simple blood test that measures your average blood sugar levels over the past 3 months. Adults ages 18 to 75 with diabetes (type 1 and 2) should have an annual hemoglobin A1C (HbA1c) test.
$35 reward for annual well-visits ages 18-19.
Your child’s physical and emotional development into adulthood is important. Well-child visits should be completed annually.

Up to $45 in rewards for substance use disorder (SUD) treatment.
Recovery is possible. If you’re struggling with addiction, call our behavioral health team toll-free at 1-800 873-2246, TTY 711. Complete a minimum of three substance use disorder (SUD) treatment sessions and you could earn rewards.1
$15 reward for beginning SUD treatment.
Start treatment today. Complete one session2 within 14 days of a new SUD diagnosis.
$30 reward for continuing SUD treatment.
Staying substance-free takes effort. Attend two more sessions2 within 34 days of your first visit (a total of three sessions).

2A session is any visit that addresses your SUD diagnosis, including a medication dispensing event (one prescription of an amount lasting 30 days or less), or a medication administration appointment.
